Forms needed for participation in the Weston Cup & Showcase: 

 
 
 
For all international teams you will need the following:
 
-       Permission to travel and participate in the Weston Cup & Showcase from your FIFA affiliated organization. For example, your country’s soccer federation or your local governing association.
-       Proof of medical insurance for players and coaches.
-       Roster of all players and coaches verified by your club. Click here for roster form (pdf) - FOR INTERNATIONAL TEAMS ONLY. Six (6) copies will be needed for tournament registration/check-in.
-       Passport for players and coaches (will serve for age verification for players).
-       Player and coaches passes. You can either use player/coaches passes issued by your governing body, as long as they include the name, date of birth and picture of the player or click here for player/coaches player passes form (pdf).
-       Medical Release Form. Click here for a medical release form. Note, for international players the form does not have to be notarized.
